Social prescribing is a non-medical approach to improving health and wellbeing, by exploring local activities and services in the community. It is designed to support people with a range of social, emotional and practical needs, and gives people to time to focus on “what matters to them”.
How we support people
- Establish a support plan and identify actions to be completed.
- Coach and empower people to recognise and make healthy choices.
- Assist people to engage with activities and services in their local community.
- Provide short-term intervention with a view to linking people to longer-term solution
