Why projects matter for our Primary Care Network

Projects are at the heart of how Primary Care Networks (PCNs) like Chesterfield & Dronfield drive meaningful change for our community. By working together on targeted initiatives—whether improving access, tackling health inequalities, or piloting new models of care—we can respond rapidly to local needs and deliver better, more joined-up services than any single practice could achieve alone. Projects enable us to trial innovative approaches, share learning, and build multidisciplinary teams that reflect the needs of our population. They help us secure additional resources, strengthen our workforce, and ensure we’re delivering care that is proactive, patient-centred, and sustainable for the future. Through collaboration and continuous improvement, our projects make it possible to address challenges, deliver national priorities, and improve health outcomes for everyone in our network.
