You have been given this leaflet because you have recently experienced the loss of a loved one who lived in a care home that the Ageing Well Team supported.
We would like to offer our condolences. Our thoughts and sympathies are with you at this difficult time. This leaflet will briefly outline what to expect following a bereavement, which is often the most difficult time for relatives/friends. We will make every effort to ensure that this process is as seamless as possible.
When the Ageing Well Team has been notified of a death by the care home, our service will arrange for the completion of the Medical Death Certificate and for this to be sent to the Medical Examiner’s Service. It is a legal requirement for all deaths to be scrutinised and this gives you an extra opportunity to raise any concerns or compliments you may have around your loved one’s care. The Medical Certificate will be sent to the registrar from the Medical Examiner Office. In some cases, the death will need to be discussed with the Coroner’s Office. If this is necessary, the reasons behind this will be discussed with you and we will explain what to expect. This will sometimes result in a small delay in getting all the necessary paperwork completed. We aim to complete the medical certificate and send it on to the medical examiner within 5 days, who will then advise you to make an appointment with the registrar for registration of death.
